Line 6: ID Number. Your California Driver’s License or ID card number is requested.

Line 7: Choice of Party. You must register with a party if you want to take part in that party’s primary election, caucus, or convention.

Line 8: Race or Ethnic Group. Leave blank.

Line 9: Signature. To register in California you must:

1) be a citizen of the United States; 2) be a resident of California; 3) be at least 18 years of age at the time of the next election; 4) not be imprisoned or on parole for the conviction of a felony; 5) not currently be judged mentally incompetent by a court of law.
